The Ontario Government wants to hear from you as they are planning major changes to the child welfare system!

If you are a youth in/from care (any age), share your advice and recommendations through an anonymous online survey, open until October 11, 2019.

How do I complete the survey?

Click the button above to complete survey on your cellphone or computer. Scroll to the bottom of the page and click the arrow    to start the survey then select ‘youth or former youth’.

You can also request a paper version by emailing: CYFSA@ontario.ca

Who is eligible in the ‘youth’ category to complete the survey?

A youth or former youth (any age) who is receiving, or has received in the past, services from a children’s aid society or who was provided with licensed residential services.

If I do not have a computer, where can I find a one to complete the survey?

Suggestions: Public libraries, High School computer labs, university/college campus, YMCA and other community centers, Employment Ontario, drop-in centers, shelters

If I do not have Wi-Fi, where can I find free access to complete the survey?

Suggestions: Public libraries, subway stations, malls, restaurants (such as McDonalds and Tim Hortons), Telus Mobility for Good Program provided by the Children’s Aid Foundation of Canada

How do I know I have successfully submitted the survey?

On the final page, make sure you click the check mark to submit your answers. If you exit the page beforehand, you have not submitted your survey and will have to complete it again.
